Current MHS student moving but staying within the Middletown High School District:
-Utility Bill (Water, Gas, Electric or Cable)
-Tax Bill
-Signed Lease (may not be expired)
-PDF copy of bill must be full first page showing parent/guardian's name,
address. and dated withing last 2 months
- If the parent and child live with someone else - owner/lessee of the home needs to provide a proof of their residence (see above list of POR options), and parent/guardian must complete FCPS Parent Residency Affidavit (Multi-Family Disclosure Form) and have it notarized that the child resides at that address.
